Upon perfecting the appeal in a civil case, the appellant must file a docketing statement in the appellate court. See Tex. R. App. P. 32.1 & 32.2.

After completing the form, in order to create a PDF suitable for electronic filing, you must use the "Print to PDF" feature as follows:
