Notice of & Assignment of Related Cases
Local Rules of the First and Fourteenth Courts of Appeals require the following statement be attached to the notice of appeal filed with the trial-court clerk. A copy of the notice of appeal and this statement must be filed with the Clerk of the First Court pursuant to Texas Rule of Appellate Procedure 25.1(e).

Docketing Statements
Printing Docketing Statements to PDF
After completing the form, in order to create a PDF suitable for electronic filing you must use the Print to PDF feature as follows:

When printing, select “Adobe PDF” as your printer destination; you will be prompted to save the file.

Filing Docketing Statements
Once the trial–court clerk has assigned the appeal to the First Court, the appellant must file a docketing statement with the Clerk.

Mediation
If the Court refers a civil appeal to mediation, three forms must be filed with the Clerk at appropriate stages in the mediation.
Information Sheet
Once the trial–court clerk has assigned the appeal to the First Court, the trial-court clerk and court reporter must file an information sheet with the Clerk.

Sample Certificates & Affidavits
Sample forms are available below for a certificate of conference (required for all civil motions), certificate of compliance (required on motions for temporary relief in original proceedings), and certificate of service (required on all documents unless an acknowledgment of service is provided). Sample forms for affidavits of indigence are also available below for use in civil cases.
